The City requires all food facilities to dispose of waste daily or as needed to avoid unsanitary conditions. Check the option below and provide the requested information:*

		My business generates three or fewer 32-gallon commercial trash receptacles per week.
		My business generates more than three 32-gallon commercial trash receptacles per week, and has a contract with a licensed waste hauler.
		My business generates more than three 32-gallon commercial trash receptacles per week and has a small hauler license for proper disposal.
